Creating an Anonymous Survey is very simple.
While building your Survey form in Webform builder, on the Configuration step, simply pick "Anonymous Survey" from the "Match responders to" list.
When you come to Step 3: Builder, the system will automatically remove the "locked" items of First & Last name, and Email address, allowing you to create a survey without these items:

Q. Can I add the Names and Email fields back into an Anonymous Survey?
A. No, you cannot collect any Personal Identifying Information (PII) about the respondent while the survey type is Anonymous. To collect PII, you'll need to change the survey type back to "Student".
Q. Does it create anonymous student records?
A. No record is created in Student Database, and all respondents' details are anonymised in the Surveys app by changing their name to "Anony Mouse".
